Transitional Care Services

Transitional Care Services at The Hartman House is an additional tier of care tailored to provide structure and support for clients in need of a higher level of care. Transitional Care services are individualized to each client’s specific needs. Through Hartman House Transitional Care, attention is given to specific elements of the after-care plan related to the adjustment from intensive in-patient or residential treatment programs to independent living in recovery. This is accomplished through the following:

  • Personal daily contact
  • Medication Management – Individual Transitional Care sessions (frequency based on need)
  • Collaboration with family and clinical teams
  • Medical/dental referrals and appointment assistance – Intensive and ongoing employment/volunteer assistance – Ongoing contact and consultation with primary treatment program
  • Interfacing with medical/mental health care professionals
  • Introductions to 12-step related meetings and strong men in recovery – Daily living skills support
  • Projected and actual budgeting
  • Random drug and alcohol screening

This is an overview of Transitional Care Services at The Hartman House. Due to the fact, Transitional Care is individualized based on personal and realistic goals for each client, an overview will not capture all the specific services provided. Clients are assessed and the program is tailored to meet their needs.
The cost for Transitional Care Services is $2,100.00 per month. If the resident is engaging and benefiting from the services, we will discuss extending the services.
