Transitional Care Services
Transitional Care Services at Hartman House offers an enhanced level of care designed to provide structure and support for clients requiring more intensive assistance. These services are customized to meet each client’s unique needs. Attention is given to specific elements of the after-care plan that facilitate the transition from residential treatment to greater independence while maintaining structure and accountability. This includes support while living at Hartman House and attending partial hospitalization programming (PHP) or intensive outpatient programming (IOP), as well as during the shift from any other treatment programming to employment, volunteering and/or school. Transitional Care may be introduced to prevent a failure to launch when a client arrives to Hartman House, or as a measure throughout a client’s residency when additional support needs are identified. This is accomplished through the following:
- Consistent contact with Hartman House leadership to provide ongoing support and coaching
- Medication management
- Individual Transitional Care sessions, frequency based on each resident’s needs
- Collaboration with both family members and clinical teams to ensure comprehensive care
- Medical, dental and therapeutic referrals, along with assistance in scheduling and attending appointments
- Extensive support in employment search, employment management and/or volunteer opportunities
- Continuous communication and consultation with primary treatment programs
- Introduction to 12-step or other recovery support meetings and recovery communities
- Assistance with daily living skills to promote independence
- Guidance in budgeting and finances (TrueLink card account management)
- Weekly Transitional Care community meals (complimentary)
- Weekly Transitional Care outing (movies, bowling, go cart racing, drive shack, mini golf, etc. complimentary)
- Reasonable transportation provided
This overview of Transitional Care Services at Hartman House provides a general idea of our approach. Our services are highly individualized to meet each client’s unique needs and realistic goals which are identified during our assessment process with each client, family and clinical team.
Our Transitional Care Services operate on a month-to-month basis at an additional cost to the standard program, with the ultimate goal of helping clients gradually reduce their reliance on this level of care. As clients progress through our program, we aim to empower them to achieve greater independence and sustain their recovery journey.

Recurrence Response
In the event a resident experiences a recurrence of their substance use disorder (relapse or setback), Hartman House has a comprehensive process to provide the necessary attention and care to address their current condition and prevent further decline. When these incidents occur, our focus is on the present and future rather than dwelling on past decisions. We are committed to never abandoning a client or creating an unnecessary crisis for their family. Our intention is to ensure their safety.
Upon intake to Hartman House, our clients sign a Recurrence of Use Agreement, which allows us to transport them to a detox facility or hospital if they are at high risk or in an acute state. During these situations we collaborate closely with the resident’s treatment team and family to determine the appropriate next steps. This may involve a temporary stay at a detox facility, return to treatment, and/or revised care at Hartman House depending on the the resident’s specific needs and history.
If the incident occurs at night, we will often stay overnight in the office with the at risk resident to ensure their safety. Our priority is to provide a supportive and safe environment to help our clients navigate these challenging moments and continue on their recovery journey.
